Abstract;In the guideline of the United Kingdom Dermatological Society, it has been specified that "first selection for nail ringworm is lamisil (L), and second selection is itrizol (I)". In Japan, placing importance on "equity", the order of selection is not often told, but it is embarrassing by the clinical decision, and there is no contribution to the evidence recognition. In the comparison of the continuous use of L and I, the superiority of L was shown with impact that in mycological therapeutic ratio, L was 3.5 times more effective than I, and in the reccurence ratio, L was about 40% of I. At present, it is the actual state that both in randomized comparative test (RCT) and cohort study, "there are evidences which show the superiority of L, and there is no evidence which shows the superiority of I". The research which sounded alarm bell on the hepatic disorder by L gave a very strong influence, but there are some problematic points that liver disease patients from earlier such as alcoholic hepatitis patients, and mental disorder patients, for whom the combined drug or the compliance are the problems, were included in the examples for the generation of side effects such as hepatic disorder. The combined use of I is a problem in the disease with a high prevalence such as hypertension, heart disease, gastric ulcer and insomnia. In either L or I, the element of "technique of the doctor" is important for early discovery of noxious phenomenon.
