Abstract; The standard for hemoglobin (Hb) abnormal value, the response when an abnormal value is observed, and the relation between panic value and disease state, are outlined. The clinical significance of extremely abnormal values (extreme values) or panic values of Hb, including high values (over 20g/dL), low values (below 5.5g/dL) and .DELTA. panic values (over 3g/dL lower than the previous value), are high, and it is necessary to be rapidly reported. When an abnormal value is observed, whether it is correct or not is confirmed by the observation of peripheral blood smear sample or the comparison with the previous value. The disease state which shows a high value is myeloproliferative disease (polycythemia vera, chronic myelogenous leukemia, essential thrombocythemia), and bone marrow examination is recommended to the doctor in charge as an indispensable examination. Acute/advanced anemia especially requires a quick response among the panic values, and on such an occasion, inquiry and examination with acute hemorrhage, hemolysis and bone marrow supression by anticancer drugs in mind are important.
