| Abstract;Abstract: The present investigation evaluates a unilateral fusion of the maxillary third and supernumerary fourth molar using radiography and histological examination. The supernumerary fourth molar had the occlusal surface with two cusps and was smaller in size than the maxillary third molar. The fusion of the teeth involved roots only, in which case cementum and dentin were shared. The pulpal communication between both teeth was recognized and had a common root with at least two root canals. The stage of development of the supernumerary tooth didn't appear to be delayed in the dentine structure compared to similar teeth of the normal dentition. Furthermore, the supernumerary tooth was located on the distopalatal side of the third molar. Thus, the fused tooth in the present study seems to have resulted in the fusion between the third molar and the supernumerary molar which might have been derived from secondary tooth germ not reduced during the development.
