| Abstract; We identified 534 pre-school-aged children (5 years old) as having WPW syndrome among 841, 881 children by a surface EGG during a heart-screening program in kindergarten from 1969 to 1991. 124 children have been followed for more than ten years. No sudden cardiac death occurred in this group, and 19.6% had supraventricular tachycardia (SVT) and 63% had only the WPW EGG pattern. Next, we reported 54 symptomatic patients aged 21 days to 16 years with clinically diagnosed WPW syndrome as a function of initial symptom status and mean follow-up of 14 years. No sudden death occurred in any of these 54 patients, atrial fibrillation occurred in one, syncope in two. One case had early disappearance (symptomatic infants who become asymptomatic after first year of life) and late recurrence. Twenty percent of the patients had become asymptomatic without surgery, while 60% had needed some treatment. There is great difference between prognosis in symptomatic and asymptomatic patients.
