Abstract;A 53-year-old man was admitted to our hospital with the chief complaint of dyspnea. He developed hypoxemia and hypertension. Chest radiograph showed cardiomegaly with congestion in the bilateral lung fields. An electrocardiogram revealed rapid atrial fibrillation. Echocardiography showed a diffusely severe hypokinesis in the left ventricular wall with hypertrophy and dilatation of left atrium, ventricle, and inferior vena cava. By these findings and further investigation, we diagnosed acute heart failure due to hypertensive heart disease with rapid atrial fibrillation, presenting low cardiac output, pulmonary congestion, and volume overload. Continuous intravenous infusion of milrinone, which is a phosphodiesterase III inhibitor with positive inotrophic and vasodilating activities, was effective for hemodynamic support and symptom reduction without worsening of tachycardia, hypertension, ventricular arrhythmia, and without occurrence of hypotension.
