| Abstract;We report a case of pregnancy complicated by essential thrombocythemia (ET) successfully treated with interferon-.ALPHA. (IFN-.ALPHA.). The patient, 28-year-old woman, was diagnosed with ET at 22-years of age and had a history of two miscarriages with no treatment for ET. Aspirin (100mg/day) was started two months prior to the present pregnancy. At the 6th week of gestation, because her platelet count was 186.9*104/.MU.l IFN-.ALPHA. was started at a dose of 21 million units (MU) per week, and later increased to 36 MU per week. At the 24th week of gestation, the leukocyte count, Hb and platelet count were 4,000/.MU.l, 8.7g/dl and 23.6*104/.MU.l, respectively. IFN-.ALPHA. was tapered off to 6 MU per week. At the 38th week of gestation, she had an uncomplicated vaginal delivery with a platelet count of 33*104/.MU.l. The infant was a healthy female weighing 2,720g. No abnormal findings were observed in the placenta. The platelet count of cord blood was 28.4*104/.MU.l. Our case provides additional information indicating that IFN-.ALPHA. is clinically effective in preventing recurrent abortion associated with ET, with no significant adverse effect. (author abst.) |
