Abstract;The effects of combined spinal epidural analgesia (CSE) on fetal heart rate (FHR) patterns were prospectively studied. Forty cases without complications were enrolled. Transient pathologic FHR was noted in 5 cases, 13% (severe variable deceleration 4, prolonged deceleration 1) within 30 minutes after intrathecal administration of 2.5 mg bupivacaine with 25 .MU.g fentanyl. One of them was associated with uterine hyperactivity, and two with maternal hypotension. No remarkable FHR changes were noted during continuous epidural infusion of 0.0625% bupivacaine with 0.0002% fentanyl and 1:250,000 epinephrine. Close fetal surveillance seemed to be mandatory especially after spinal analgesia. (author abst.)
