Abstract;Endoscopic procedures have been widely employed in various fields of surgery. However, endoscopic neck surgery has not become a common surgical procedure due to the difficulty in creating a working space. The large scar on the anterior neck wall placed at the front of the patient by the conventional technique is clearly visible, which is a disadvantage from the cosmetic view point. Taking into account cosmetic considerations, we performed endoscopic thyroidectomy on a 59-year-old female with a thyroid benign tumor at the left lobe of the thyroid. The endoscopic thyroidectomy consisted of three steps. The first was to create a working space below the subplatysma muscle lifting the anterior neck wall with a subcuit dissector. The second step was exploration of the thyroid gland dividing between the sternocleidomastoid muscle and anterior cervical muscle. The third step was partial thyroidectomy using a Harmonic Scalpel without injury to the recurrent nerve. The operating time was 140 min. and the blood loss was 36 ml. No complications developed during or after the surgery. The resultant scar was small and could be covered by clothes, so that the cosmetic result was excellent. Endoscopic thyroidectomy provides a minimally invasive procedure and can yieldmade good results cosmetically. (author abst.)
